The implementation of DC CareLink facilitates DBH's access to comprehensive data on needs in our communities.  It helps organizations to identify and address possible gaps more effectively, leading to better overall outcomes for the populations in need. By streamlining operations and taking advantage of the CCL’s software secure and efficient solutions, communities can ensure that they are providing clients with the best possible care through referral programs. It also gives agencies and service providers crucial data that can impact their fundraising efforts.

 

DC CareLink's goal is to be the platform that Empowers and Connects the District’s Healthcare Continuum which is enhanced when service providers become network partners.  Network partners regularly communicate, collaborate resources, share social trends and success stories, discuss issues and concerns that arose and the resolutions and learn from each other or ask for solution from additional resources to fundraising.  To become a Network Partner, please go to our Contact Us form and check the box "My Agency/Organization is interested in becoming a Network Partner."

There are different levels that Agencies and COBs can participate to actively share DCCareLink with District residents:  ​

 

1. Utilize the Resource Guide by embedding a link on your website

2. Access the online Referral Link:  https://www.dccarelink.org/enter-client-referral and enter a referral for a client who is seeking services.

3. To fully participate and become a Network Partner, complete the form:  https://adasignup-g8cccmcafmhdgwap.canadacentral-01.azurewebsites.net. In the Notes section please indicate your interest in becoming a Network Partner, with the decision maker's contact and availability to meet with our Community Outreach team.
