Who We Are...

DC CareLink connects DC residents to essential social services through a unified network of community organizations and behavioral health providers. Led by the Anacostia Coordinating Council and funded by the DC Department of Behavioral Health, we provide a citywide resource guide and integrated case management system to help individuals and families thrive.

Resource Guide

The DC CareLink Resource Guide is a centralized, searchable directory of community-based organizations and services. It can be implemented on any website where people may be searching for assistance, enabling providers and community members alike to quickly locate resources in their area. The Resource Guide strengthens care coordination and improves access to support.

Social Service Case Management Technology

The District is fortunate to have Community CareLink as its technology partner. Community CareLink (CCL) is an online case management system designed for the unique programs of social services organizations.  CCL functionality includes client demographic entry, easy case note entry, scheduling, custom forms, social service assessments, referrals to agency partners, and many other functions.

Partnering with CCL allows DC CareLink to enable seamless communication, data sharing, and program outcomes across the community.  The CCL system cost is covered for any qualified organization in the District during the lifetime of the DC CareLink program.
